Community Wellness Hubs

Our Community Wellness Hubs in Georgetown and Oakville are welcoming, community‑based spaces that bring health, social, and wellness services together in one convenient location. Operated by Links2Care, these hubs are designed to help older adults and adults with disabilities live well, stay connected, and remain safely in their homes and communities.

Supporting Well‑Being Close to Home

Through a single point of access, our Community Wellness Hubs make it easier for residents to find the right services at the right time. Our team works directly with individuals and caregivers to provide:

  • Information and service navigation
  • Referrals to local supports
  • Advocacy
  • On‑site programs that promote overall health and well‑being

Coordinated, Person‑Centred Support

Services at our Georgetown and Oakville hubs are delivered in partnership with local health and social service organizations, ensuring coordinated, person‑centred care. By bringing essential supports together in one place, our Community Wellness Hubs help people stay connected, maintain independence, and access the resources they need, right in the communities where they live.
