Position Summary:
Older adults transitioning out of shelters to permanent housing often require additional support for managing medical comorbidities and mental health issues, particularly if they lack established social and family networks. These individuals frequently struggle with maintaining stable housing due to insufficient support and diverse health challenges. Offering comprehensive, wrap-around services that address both health and social care needs can significantly improve the likelihood of successful and sustained housing outcomes for these individuals.
As the Navigator within the Navigate Halton transitional program, you will support the individuals in their transition from shelters to stable housing. Your primary objective is to facilitate independent living by addressing their complex needs, which may include, but are not limited to, medical, financial, mental health, and substance use disorders. Utilizing the Housing First Model and a trauma-informed approach, you will ensure a supportive and respectful environment that acknowledges their experiences and promotes empowerment.
Key Responsibilities
• Is knowledgeable on and uses a housing first approach that to focuses on providing supports through case management.
• Ensures a wrap around, person centered approach to the service delivery which will address mental health, physical health, substance misuse and housing retention.
• Uses a trauma informed care approach to service delivery that recognizes and responds to the impact of trauma on individuals.
• Ensure individuals have access to primary care, mental health services, specialists, medication management, and other necessary health supports.
• Initiate/attend case management meetings with professionals involved with individuals ensuring collaborative service delivery.
• Provide or broker assistance to improve self-sufficiency including but not limited to
• Life skills development, and social integration (e.g., cleaning, cooking,) providing assets to the individual to support future permanent housing.
• advocate and liaise with mental health services, substance abuse programs, legal institutions, and government agencies
• Facilitation of client involvement in community activities, recreational opportunities, and educational supports
• Connection to community supports such as food security and social support groups
• Assist with transportation and/or connect to transportation service for medically necessary supports.
Qualifications
• Bachelor’s degree/Diploma in Social Work (preferred), Geriatrics, Community Development, or other related field combined with a minimum 1–2-year relevant experience working with seniors and individuals with disabilities. Previous experience supporting persons in crisis and/or with complex needs is required.
• Outstanding verbal communication skills and written skills.
• Demonstrated ability to assess with good judgment and instinct in understanding and supporting vulnerable populations and the needs of clients.
• Thorough understanding and awareness of issues impacting seniors and persons with disabilities including homelessness, poverty, abuse and mental health.
• Sound knowledge of community resources and programs available to seniors and persons with disabilities in Halton and Mississauga. Knowledge of the programs and services provided by Links2Care is an asset.
• Working knowledge of, as well as seniors programming, funding sources and implications of program decisions.
• Strong organizational and administrative skills.
• Proficient computer skills in Microsoft Office. Comprehension of the InterRAI Cha client assessment tool, COTA and Alayacare is an asset.
• Outstanding documentation skills with the ability to create accurate and detailed client case notes.
• Tact, discretion, honesty and ability to develop positive relationships with seniors and adults with disabilities.
• Ability to safeguard and maintain a high level of confidentiality.
• Ability to establish and maintain professional boundaries and work within a defined scope of service.
• Ability to work independently with little direct supervision.
• Completion of a satisfactory Criminal Reference Check and Vulnerable Sector Screen prior to and throughout employment is required.
• Current First Aid/CPR , Non-Violent Crisis Intervention, Assist, Motivational Interviewing, Housing First Model an asset.
• Must have reliable transportation, a valid Ontario driver’s license and a minimum of $2,000,000 liability insurance coverage, in the candidate’s name.
