Position Overview
The Director of Community & Navigation Services, reporting to the CEO, is responsible for community and navigation programs. This position oversees various areas, including Wellness Hubs, Navigation Services, Navigate Halton partnerships, Children’s programs, and other community service initiatives. The Director also provides leadership to Managers to ensure high-quality services and compliance within the community services sector.
Role Accountabilities
• Comply with the Links2Care policies, standards, guidelines, and procedures
• Participate constructively in staff and other designated internal meetings
• Establish program objectives in line with organizational/contractual requirements and accountability under Ontario Health, Ministry of Education, Halton Region, Quality First and CARF accreditation standards
• Diligently work to improve the client and employee experience by understanding their needs and identifying gaps and implementing cutting edge solutions
• Create and sustain a culture of excellence and inclusivity; holding self and others accountable for individual and team development that align with the needs of Links2Care
• Constantly review and make recommendations for improvement of the organization’s policies, procedures and practices
• Develop and implement operational strategies by identifying and researching trends, issues, information, data, and reports and makes recommendations that support program and organizational vision, strategic thinking and direction
• Measure and report program/service performance and outcomes, implementing change as needed
• Utilize people strategies, including employee relations, training and development, succession planning, records management, health & safety and legislative compliance
• Support the team around accreditation requirements for all funder programs to which leadership is being provided
• Maintain the highest level of confidentiality of all pertinent Links2Care documentation/information
• Work collaboratively as a member of the Senior Leadership Team
• Lead and support the Managers/People Leaders with issues pertaining to client and employee challenges, complaints, operations, process development and policy and procedures
• Work in partnership with all team members in the planning and delivery of education and in-services pertaining to quality of care, payroll and employment standards
• Guide their team in the completion of employee, client and program evaluations
• Identify and mitigate risks through education, evaluation, advocacy and process development
• Achieve results by setting a good example, being a positive influence and promoting harmonious team spirit
• Inspire the Managers/People Leaders to maintain a climate that attracts, keeps and motivates a diverse team of top-quality people
• Act upon, in cooperation with the Managers/People Leaders, any escalated incident reports, complaints along with guiding clients and staff through the problem solving/complaints process
• Provide crisis intervention and conduct investigations as necessary
• Inform the CEO of all situations, issues or risks
• Compile and present a quarterly Quality Report based on the quality framework designed
Monitor and Evaluate
• Maintain records as per Links2Care standards
• Report in writing: all accidents, injuries and occurrences
• Provide reports as required or requested
• Collects and maintains client and employee information based on program needs and employer requirements in accordance with the Links2Care confidentiality/privacy policy, PHIPA and PIPEDA legislation and IAR and HPG privacy standards
• Maintain positive relationships with networks, professional organizations, and other related service providers
• Represent the mission, values, and philosophy of Links2Care in the community, including networks or meetings
• Assist in the training and mentoring of students, new staff, and/or volunteers
• Ability to develop relationships within the ‘circle of care’, with community partners, and funders
Professional Development
• Set annual performance and professional development goals for program and direct reports aligned to organization goals
• Attend relevant conferences, workshops, and other in-service events as approved
• Knowledge of Links2Care’s Financial and Human Resource policies/procedures
• Account for any program-related funds for which responsibility is granted
• Perform other duties as assigned
• Ensure activities meet and integrate with Links2Care’s organizational requirements for quality management, health and safety, values, and mission
• Uphold and ensure a client-centered approach all the while showing empathy and patience when interacting with clients
• Ability to work with distractions, prioritize tasks, manage complex environments and meet established timeframes
• Perform duties independently and in a team setting and demonstrate skills related to organizing and time management
• Mature personality and the capacity to maintain the highest level and standards of confidentiality
• Exceptional communicator with outstanding verbal and written skills
• Proficient computer skills comprising data management, scheduling and Microsoft Office
• Tact, professionalism, discretion, honesty and proven tract record in developing positive relationships with seniors and adults with disabilities
• Sound knowledge of the community resources available to seniors and persons with disabilities in Halton Region, Mississauga and Etobicoke.
• In depth understanding of the programs and services provided by Links2Care
• Working knowledge of the Ministry of Health and Long-term Care, as well as seniors programming, funding sources and implications of program decisions
• Establish and maintain professional boundaries and work within a defined scope of service
Qualifications
- Bachelor’s degree in related field such as Social Work, Community Development or other related degrees
- Senior leadership experience minimum 7 years in related industry
- Outstanding verbal communication skills and written skills at director competency level.
- Demonstrated ability to assess with good judgment and instinct in understanding and supporting vulnerable populations and the needs of clients.
- Thorough understanding and awareness of issues impacting seniors and persons with disabilities including homelessness, poverty, abuse and mental health.
- Sound knowledge of community resources and programs available to children, seniors and persons with disabilities in related geographical areas including Halton Region. Knowledge of the programs and services provided by Links2Care is an asset.
- Working knowledge of Home and Community Care mandate, as well as seniors programming, funding sources and implications of program decisions.
- Strong organizational and administrative skills.
- Ability to work independently with little direct supervision. Completion of a satisfactory Criminal Records and Judicial Matters Check (CRJMC) prior to and throughout employment is required.
- Must have reliable transportation, a valid Ontario driver’s license and a minimum of $2,000,000 liability insurance coverage, in the candidate’s name.
