Position Overview
The Coordinator, Home Maintenance & Repair (HMR), is accountable for coordinating the delivery and efficient operation of the HMR referral program. The Coordinator matches screened providers to clients in the community who have requested one of HMR’s services. Accountabilities include HMR provider recruitment and screening; record client work and satisfaction; accurate and timely administration.
As a referral model program, the Coordinator is responsible to provide key supports to providers who are screened by Links2Care but are independent workers and are not employees of Links2Care.
Role and Responsibilities
- The Coordinator, Home Maintenance and Repair (HMR) is responsible for the overall Coordinator operations and KPIs for the HMR program.
- Recruit, interview, select and onboard qualified providers to carry out Home Maintenance and Repair activities safely and efficiently.
- Maintain confidentiality of all pertinent client and agency documentation/ information.
- Screen new referrals of new clients to determine client goals and service eligibility.
- Coordinate services for clients, continually monitoring and adjusting service to address clients’ changing goals.
- Collect and maintain client and service provider information and run reports to ensure KPIs maintained.
- Meet employer requirements in accordance with the Links2Care policy and relevant legislation.
- Represent the mission, values, and philosophy of the agency in the community, including networks or meetings as requested.
- Take initiative in outreach and promote Links2Care programs in the community to build awareness and increase clientele.

Qualifications
- Related degree/diploma to ensure understanding services and program requirements for seniors and persons with disability with a genuine interest in providing professional, caring and compassionate service to clients.
- Working knowledge of community resources and programs available to seniors in work area.
- Strong organizational and administrative skills coupled with strong computer skills in MS office.
- Ability to safeguard and maintain a high level of confidentiality of client information.
- Ability to work independently with little direct supervision.
- Completion of a satisfactory Vulnerable Sector Screen prior to placement and throughout placement as requested by Links2Care is required.
- Staff who drive on behalf of Links2Care for work related purposes must possess and provide proof of a valid Ontario driver’s license and a minimum of $1,000,000 liability insurance on their vehicle.
- Requirement for travel in Halton, Mississauga and Etobicoke area.
