Position Overview
Older adults transitioning out of shelters to permanent housing often require additional support for managing medical comorbidities and mental health issues, particularly if they lack established social and family networks. These individuals frequently struggle with maintaining stable housing due to insufficient support and diverse health challenges. Offering comprehensive, wrap-around services that address both health
and social care needs can significantly improve the likelihood of successful and sustained housing outcomes for these individuals.
Role and Responsibilities
- Empower individuals to build basic life skills, independence, and a better quality of life.
- Work closely with Navigator to identify any concerns or issues related to the individual’s health or social well-being.
- Provide coaching to the individual that may need reminders to complete the following:
o General tidying and decluttering of living space
o Washing floors and vacuuming of carpets
o Cleaning of washrooms
o Occasional cleaning of the inside of the refrigerator
o Cleaning of the top and front of the stove
o Dusting, polishing furniture, cleaning mirrors if required
o Laundry
o Removal of garbage
o Making and keeping of appointments
o Completing Bed Bug Protocol monthly - Assist with appointments and correspondence and support health care routines.
- Maintain awareness of individual abilities, current diagnosis made by medical professionals, diet, personal interests and special needs
- Participates and collaborates with a multidisciplinary care team (including Community Navigation team, individual and caregivers) in supporting the individuals needs according to their care plan.
- Actively listens, providing non-judgmental support to individuals for self-directed decision making
Qualifications
- Supportive Care Worker or Community Service Worker Certificate or Health Care Aide Certificate or completion of a PSW program or equivalent related years of experience
- Current CPR, First Aid Certificate and/ or Mental Health Certificate an asset
- Non-Violent Crisis Prevention, Assist, Motivational Interviewing and knowledge of House First Model is an asset.
- Effective communication skills, written and verbal
- Ability to interact with others in a caring, tactful and courteous manner
- Excellent time management skills with the ability to work independently with minimal supervision.
- Ability to work collaboratively and cooperatively with others as part of a team.
- Completion of a satisfactory Criminal Records and Judicial Matters Check (CRJMC) prior to employment and throughout employment as requested by Links2Care is required.
- Valid driver’s license as well as a minimum of $2,000,000 liability insurance on their vehicle.
